First Visit
What to Bring
To make your first visit as smooth as possible, please bring the following:
- Completed new patient forms (or arrive 15 minutes early to fill them out)
- Photo ID for the parent or guardian
- Insurance card(s)
- Immunization records (if available)
- Previous medical records or specialist notes, if applicable
